IPS de famille serves a predetermined territory in the southwestern part of the island of Montreal.

Specifically, postal codes beginning with : H3E, H3J, H3K, H3X, H3Y, H3Z, H4A, H4B, H4C, H4E, H4G, H4H, H8N, H8P, H8R, H8S, H8T.

If you are out of the area, appointments can only be booked by calling (438) 951-2747.
Additional travel charges may apply.


TYPE OF APPOINTMENT

Consultation fees are not reimbursed by the Régie de l’assurance maladie du Québec (RAMQ) and are therefore at the patient’s expense. A receipt for professional services rendered will be issued for reimbursement by private insurers and/or for reimbursement of medical expenses when filing income tax returns. It is the patient’s responsibility to verify his or her own coverage with his or her private insurer, if applicable. There are no opening fees or annual membership fees. Each consultation appointment is payable individually.


EXCLUDED SERVICES


Certain services and reasons for consultation are excluded from the IPS de famille service offer. More specifically, these are consultations for:

  • Accidents at work (CNESST) and on the road (SAAQ)
  • Pregnancy follow-up
  • Follow-up of mental health conditions
  • Follow-up and renewal of prescriptions related to mental health, narcotics and benzodiazepines
  • Follow-up of work stoppage, sick leave or disability initiated by a health practitioner other than via IPS de famille
  • Medical aid in dying
